Instructions to complete the complaint form

Instructions on how to complete the form are in the Guide and in the Preparing your complaint form documents. Add extra pages if you need more space to answer questions, up to a limit of 20 single-sided pages, including the form. List documents (if you have any) that relate to the complaint but do not send them. Documents may be requested later in the complaint process.

The complaint form and guide is available in two formats:

1. Fillable PDF (12-point font)

You can download the complaint form and guide to your computer. Enter your information on the form and save it.

OR

Print the form and guide and write your complaint on the form.

2. Large print fillable PDF (24-point font)

Upon request, the Commission will email or mail you the large print complaint form and guide.

Submitting your complaint form

Completed forms can be emailed, faxed, mailed or dropped off at a Commission office. A Human Rights Officer will assess your complaint and, in time, they will advise you whether your complaint is accepted.

Please note, if you are printing your complaint form, print the form on one side only, leaving the back pages blank. This also applies to any additional pages.
